Self Management Programme Workers
(Long Term Conditions, Older People and Carers)
18.5 Hours per Week – Fixed Term until March 2014
Salary: £18,402 - £21,798 pro rata
We are looking to recruit 6 Self
Management Programme Workers (Long Term Conditions, Older people and Carers) to
work across the region to support and deliver self management activities for
individuals and groups. Supported by the Putting You First’s Change Fund each
post will be based in one of 6 locations across Dumfries and Galloway: -
Dumfries, Annan, Kirkconnel, Castle Douglas, Whithorn and Stranraer. As part of
Building Healthy Communities Self Management programme the post holder will
promote health and wellbeing through empowerment of individuals and
communities, partnership working and outreach via locally led self management initiatives.

Salary: £14,258 - £17,003 pro rata Band 2
